Introducing Deltacortril, a powerful glucocorticosteroid designed to effectively modulate the activity of leukocytes and tissue macrophages. This medication plays a crucial role in limiting leukocyte migration to inflamed areas and inhibiting macrophage functions such as phagocytosis and interleukin-1 production. Additionally, Deltacortril stabilizes lysosomal membranes, reducing the concentration of proteolytic enzymes during inflammation, and decreases capillary permeability caused by histamine, while also inhibiting fibroblast activity and collagen synthesis.
By inhibiting phospholipase A2, Deltacortril reduces the production of prostaglandins and leukotrienes, as well as suppressing COX enzymes (particularly COX-2), leading to a significant reduction in prostaglandin synthesis. It decreases circulating levels of lymphocytes (both T and B cells), monocytes, eosinophils, and basophils by directing them from the bloodstream into lymphoid tissues and suppressing antibody formation.
Deltacortril also affects hormonal release, reducing ACTH and beta-lipotropin from the pituitary gland without altering beta-endorphin levels, while inhibiting TSH and FSH secretion. Its vasoconstrictive effects are notable when applied directly to blood vessels.
On a metabolic level, Deltacortril has significant, dose-dependent impacts on carbohydrate, protein, and fat metabolism. It promotes gluconeogenesis and enhances amino acid uptake in the liver and kidneys, stimulating gluconeogenesis enzyme activity. In the liver, it facilitates glycogen storage and glucose synthesis from protein metabolism products, leading to elevated blood glucose levels and increased insulin secretion.
While Deltacortril inhibits glucose absorption in fat cells, it simultaneously promotes lipolysis, resulting in fat accumulation due to the surge in insulin secretion. Furthermore, Deltacortril demonstrates catabolic effects on lymphoid and connective tissues, muscle, adipose tissue, skin, and bone. Compared to hydrocortisone, its influence on water and electrolyte metabolism is milder, promoting potassium and calcium loss while retaining sodium and fluid. Long-term use may lead to conditions such as osteoporosis and Cushing's syndrome, and it may inhibit growth in children.
At higher doses, prednisone can increase brain tissue excitability and lower the convulsive threshold, as well as boost hydrochloric acid and pepsin production in the stomach, raising the risk of peptic ulcers.
Deltacortril provides substantial therapeutic benefits through its anti-inflammatory, antiallergic, immunosuppressive, and antiproliferative properties. When used topically, its effectiveness stems from its anti-inflammatory and vasoconstrictive effects.
Notably, Deltacortril's anti-inflammatory potency is four times greater than that of hydrocortisone, while its mineralocorticoid activity is approximately 60% less.
Indications for Deltacortril:Prescribed for oral and parenteral administration, Deltacortril is suitable for treating conditions such as rheumatism, rheumatoid arthritis, dermatomyositis, periarteritis nodosa, scleroderma, ankylosing spondylitis, asthma, acute and chronic allergic reactions, anaphylaxis, Addison's disease, adrenal insufficiency, liver conditions, lipid nephrosis, agranulocytosis, various leukemias, lymphomas, thrombocytopenic purpura, hemolytic anemia, chorea, and various skin disorders including pemphigus, eczema, psoriasis, and systemic lupus erythematosus (SLE).
For intra-articular administration, it is effective for chronic arthritis, post-traumatic arthritis, and osteoarthritis in larger joints. Tissue infiltration is indicated for conditions such as epicondylitis, tenosynovitis, bursitis, frozen shoulder, and Dupuytren's contracture.
In the field of ophthalmology, Deltacortril is utilized for allergies, chronic conjunctivitis, blepharitis, and corneal inflammation.
Dosage and Administration:For adults on replacement therapy, the starting oral dose is 20-30 mg, with a maintenance dose of 10-15 mg daily. Initial doses may range from 15-100 mg per day, tapering down to maintenance doses of 5-15 mg daily. For children, a starting dose of 1-2 mg/kg divided into 4-6 doses is recommended, with maintenance doses of 3-6 mg/kg per day.
For intramuscular or intravenous administration, dosing frequency and duration are tailored to individual needs. For intra-articular administration, 25-50 mg is recommended for larger joints, 10-25 mg for medium joints, and 5-10 mg for smaller joints. In ophthalmology, Deltacortril is typically administered three times daily for up to 14 days; in dermatology, it may be applied once to three times daily.
Potential Side Effects and Adverse Reactions:Endocrine effects may include menstrual irregularities, adrenal suppression, Cushing's syndrome, and growth retardation in children. Digestive issues can range from nausea and vomiting to peptic ulcers and gastrointestinal bleeding. Metabolic effects may include protein catabolism, increased calcium excretion, and weight gain.
Cardiovascularly, it can lead to potassium loss, arrhythmias, and elevated blood pressure. Musculoskeletal effects may include inhibited growth in children and osteoporosis. Central nervous system reactions can range from headaches to mood fluctuations.
Vision-related risks include cataracts and increased intraocular pressure. Dermatological effects may manifest as skin thinning, bruising, and increased susceptibility to infections. Immunosuppressive reactions can slow healing and increase infection risk, with rare cases of allergic reactions or complications from parenteral administration.
